HIGH SCHOOL/MIDDLE SCHOOL PARTICIPATION DIRECTIONS:

Choose Oklahoma, Choose Cushing, Choose Electronic Participation Forms.

1. For athletics and marching band participation, you must print off a copy of the OSSAA physical form and take it the physician. A blank form can be found on under printable documents. The form must be filled out and signed by a parent/guardian AND physician. If you took your physical in the Spring we should have it on file. You can scan and email to [email protected].or students can give their physical form to their respective coach. Students are not allowed to begin practice without a physical (applicable activities).

2. To submit online you and your student must fill out and sign in all required spaces. Forms will not be accepted electronically without everything filled out by the student and the parent/guardian.

3. To access the required extracurricular participation forms for Cushing Public Schools, hold your cursor over the “ONLINE FORMS” tab. You will get a drop down list of all the required forms (see below).

4. You will use your child’s First Name, Last Name, and Birthdate to validate your student’s record.

5. TO SIGN THE DOCUMENT, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature.” If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using an iPad, you may create an electronic signature, by holding your finger to the screen and signing or by using a stylus pen and signing.

6. Once you have filled out all of the information on each page you will have the opportunity to print the documents. Printed documents are for your record only.

7. Forms required:
-7th-12th athletes – All forms
-7th-12th marching band – All forms
-7th-12th other activities/organizations – Drug and Alcohol Consent

Beginning this year, all students in extracurricular activities in grades 7-12 will be subject to random drug testing. An activity fee of $10 is required to pay for drug testing. This activity fee should be turned in to your student’s coach/sponsor.

NON-ATHLETES OR NON-MARCHING BAND WILL NOT TURN GREEN (IN COMPLIANCE). HOWEVER, ONCE THEY HAVE FILLED OUT THE REQUIRED FORMS THEY ARE IN COMPLIANCE.
